<p><strong>Cellulite is a lipodystrophy, changes in the skin and subcutaneous fat. It occurs as a result of hormonal disorders and leads to impaired microcirculation with lymphatic edema.</strong></p>
<p>Cellulite, which we see in everyday life, on the beach, in the mirror, is not a disease. Due to numerous publications in the media, cellulite is associated primarily with aesthetic problems, which generally require the use of ointments and creams.</p>

<h3>Skin is the outer covering of the body that protects the body from numerous external influences.</h3>
<p>The skin consists of the epidermis, dermis, and subcutaneous fat (hypodermis).<br />
The skin participates in respiration, thermoregulation, metabolism, various types of surface sensitivity (pain, pressure, temperature), and many other processes. Elastic, collagenous, and smooth muscles give the skin high strength and elasticity.</p>

Fat cells are surrounded by arterial, venous and lymphatic microcapillaries, through which lipids enter the blood and nourish the tissues of the body. There are also nerve receptors and nerve fibers, with the help of which tissue functions are regulated.</p>
<h3>There are 4 stages of cellulite development:</h3>
<ul>
<li>The first stage is edema; it is characterized by fluid retention between cells. This is a relatively “mild” form of cellulite.</li>
<li>The second stage is the compaction and hardening of connective tissue fibers. It manifests itself in the deterioration of the outflow/inflow of blood and lymph at the capillary level (the skin loses elasticity and changes color).</li>
<li>The third stage is liposclerosis. It is very often accompanied by dermosclerosis. The skin thickens, pigmentation appears. Blood circulation in the skin and subcutaneous tissue is significantly impaired. This stage is often described as a “severe form of cellulite.”</li>
<li>The fourth stage is characterized by an increase in the number of nodules that merge and are a source of pain when touched or pressed. Ulceration is possible. Quite often, the presence of infection is noted..</li>

<p>The above classification is very conditional. In neighboring areas of the skin and subcutaneous tissue, various processes can occur simultaneously. <p><strong>Trophic ulcers have been known since ancient times. They are even mentioned in the Bible. Ulcers have been found in mummies in Egyptian tombs. For centuries, people have tried to find cures and ways to treat trophic ulcers.</strong></p>
<p>This remains a problem for people and a challenge for the healthcare sector today. Trophic ulcers are very difficult to treat.</p>
<p>A trophic ulcer leads to impaired working capacity and disability. The patient cannot take a shower on his own, wear normal shoes, clothes, visit the beach, swimming pool. If you do not pay attention in time and do not start the process of treating trophic ulcers, a long-term chronic infection may begin, which will eventually lead to complications of the immune system, heart, lungs, kidneys, etc. In patients with ulcers that have not healed for a long time, there is a high risk of the ulcer degenerating into cancer.</p>

										<content:encoded><![CDATA[<h2>Treatment of lymphostasis (lymphedema) of the upper extremities</h2>
<p>Treatment of lymphostasis, lymphedema of the upper extremities, is a complex of symptoms that occurs in some patients after treatment for breast cancer.</p>
<h3>Lymphostasis of the upper limb</h3>
<h3>Postmastectomy syndrome (hereinafter referred to as PMS) consists of:</h3>
<ul>
<li><strong>lymphedema (lymphostasis, lymphedema)</strong> of the upper extremities, anterior chest wall, upper arm (develops in 35-40% of patients);</li>
<li>impaired venous outflow from the affected limb;</li>
<li>cicatricial changes of the chest wall (absence of the breast);</li>
<li>cicatricial changes in the inguinal region (due to removal of inguinal lymph nodes);</li>
<li>impaired movement function in the shoulder joint – 40% of patients suffer from limited movement in the shoulder joint;</li>
<li>damage to the brachial nerve plexus with the development of sensory and motor disorders, impaired regulation of vascular tone, polyneuropathy;</li>
<li>radiation burn of tissues (develops as a result of radiation treatment);</li>
<li>fibrosis of the tissues of the upper limb, upper arm, anterior chest wall and scapula;</li>
<li>the accession of erysipelas;</li>
<li>psycho-emotional disorders associated with the main oncological diseases and the consequences of the surgery;</li>
<li>As a result of PMEC, blood flow disorders in the neck vessels may develop, up to and including cerebral circulation disorders.</li>
</ul>
<p><strong>PMEC is distinguished by the time of onset. The early stage is at the stage of surgical treatment, the late stage develops from several months to several years.</strong></p>
<h3>Classification of PMES (upper limb lymphostasis, upper limb lymphedema)</h3>
<h4>Different manifestations of upper limb lymphostasis are classified:</h4>
<ul>
<li>1st degree – increase in limb volume up to 150 ml (compared to a healthy limb)</li>
<li>2nd degree – from 150 to 300 ml or + 1-2 cm – volume in the shoulder</li>
<li>Grade 3 – from 350 to 500 ml or + 2-4 cm of volume in the shoulder</li>
<li>Grade 4 – from 500 to 700 ml or + 4-6 cm of volume in the shoulder</li>
<li>Grade 5 – more than 700 ml and shoulder circumference more than 7 cm</li>
</ul>
<h3>Treatment of PMEC</h3>
<p>Treatment methods for PMEC can be divided into conservative and surgical. Depending on the specific lesion of certain limbs, the doctor prescribes the necessary and in each case individual treatment. Only under this condition will the treatment process be safe for the patient&#8217;s health.</p>

<p>Lymphostasis, lymphedema of the lower extremities can be divided into primary and secondary.</p>
<p>Primary lymphedema occurs either at birth or during the growth period of the body. Congenital – develops from birth. Early – begins in childhood. Late – begins in adulthood.</p>
<p>Secondary lymphedema occurs after surgery (usually oncological &#8211; when lymph nodes are removed to prevent metastases of the primary tumor. This is oncogynecology, oncoproctology, oncourology, etc.) or after radiation therapy. Lymphedema can occur after infectious diseases, such as erysipelas. There are many other different causes of lymphedema.</p>
<p>Lymphostasis of the lower extremities can be distinguished by the following signs: absence of lymphatic vessels and lymph nodes (Aplasia), very thin, insufficiently large lymph nodes (Hypoplasia), excessively dilated lymphatic channels (Ectasia).</p>
<p>It is also worth noting that the development of secondary lymphostasis of the lower extremities is negatively affected by chemical and physical factors, hormonal disorders, metabolism, excessive uncontrolled weight, infections, etc.</p>
<p>Lymphostasis of the lower extremities can be divided into unilateral and bilateral. However, there are cases when one half of the body is affected, that is, the upper and lower extremities are affected by the disease on the same side.</p>
<h3>Lymphostasis of the lower extremities and conservative treatment</h3>
<p>The outcome of conservative treatment largely depends on how quickly the patient seeks help from the onset of lymphedema. If a doctor is not consulted on time, the treatment process will be delayed and may be accompanied by complications. During conservative treatment, the following are used:</p>

										<content:encoded><![CDATA[<h2>Trophic or hypertensive ulcer of Martorell</h2>
<p>In the middle of the last century, the Spanish doctor Fernando Martorell Otzet described a trophic ulcer of the lower extremities, which he called “Martorell’s hypertensive ulcer”. He believed that its occurrence was associated with arterial hypertension. In addition, according to the doctor, the pathophysiology of hypertensive ulcers is based on impaired blood circulation in the skin.</p>
<p>When a patient is diagnosed with a trophic ulcer, or Martorell ulcer, this is an accurate definition of the type of disease.</p>
<h3>Martorell&#8217;s ulcer</h3>
<p>The location and spread of Martorell ulcers (hypertonic ulcers) has its own characteristics &#8211; most often they form on the front, outer or back of the surface of the lower legs and feet. This disease mainly affects women over 40 years of age. At the initial stage of development, shallow ulcers are observed, without visible granulation, minor wounds, in places there are areas of black dermis necrosis. If you do not pay attention to these signs in time and do not consult a doctor, the size of the ulcers will increase, a secondary infection will join them.</p>
<p>However, the most characteristic feature of hypertensive ulcers among other types of lower limb ulcers is the intense pain syndrome: This pain causes severe suffering for the patient. Even a minor dressing turns into a painful procedure for the patient. There are cases when the pain syndrome cannot be removed or weakened even by strong analgesics. During my practice of treating Martorell ulcers, I have met patients who had suicidal thoughts, which arose as a result of prolonged and continuous pain. Because of this pain, patients are deprived of normal sleep, they develop significant psycho-neurological disorders.</p>
<p>During this disease, pathophysiological changes in the vessels are also observed, which can be detected during duplex examination; this examination can also show whether there is damage to the main arteries and veins. The diagnosis of Martorell&#8217;s ulcer is also confirmed by microscopy of a biopsy of the skin and subcutaneous tissue, which reveals hyalinosis of the walls and endothelial proliferation with complete disappearance of the vascular lumen.</p>
<p>Martorella&#8217;s trophic ulcer in its &#8220;pure&#8221; form, as described by the Spanish doctor Fernando Martorella, is not very common. Patients, in addition to arterial hypertension, may also have concomitant pathology &#8211; varicose veins, obliterating atherosclerosis, diabetes mellitus, overweight, etc. If concomitant pathology is added to Martorella&#8217;s ulcer, the diagnosis and treatment of arterial ulcers becomes even more difficult.</p>
<h3>The treatment process of Martorell&#8217;s trophic ulcer</h3>
<p>Treatment of hypertensive ulcers is an extremely difficult task. Even the slightest touch to the surface of the ulcer during bandaging can cause the patient not only discomfort, but also unbearable suffering. All medications and ointments that are applied to the surface of the ulcer only cause a sharp increase in pain. Depending on the stage of the disease, the drugs used to treat the ulcer are selected. Sometimes it is necessary to change several dozen ointments, creams, and powders to achieve a positive result. During the treatment of Martorell ulcers, or Martorell syndrome ICD 10, it is necessary to correct blood pressure.</p>

<p>There is no person who has not seen vascular meshes or stars on their legs or face &#8211; this is nothing more than varicose veins of the subcutaneous veins.</p>
<p>The features of the occurrence of both varicose veins and spider veins or reticular veins are the same &#8211; weakness of the venous wall and increased pressure in the veins. In women, reticular varicose veins are much more common, which can be explained by pregnancy and childbirth. It is also worth noting the occupational hazard, in particular, this disease can affect surgeons, teachers, salespeople, flight attendants, etc., that is, those people who spend a significant part of their working time in an upright position, especially if there is a genetic predisposition to varicose veins. Similar changes can appear with age, for example, blue-violet veins in the skin (on the thighs and calves) increase. Increased pressure in small veins that pass to the subcutaneous tissue and skin leads to their increase, both in diameter and in length. These veins begin to twist, curl. This happens because the pressure in the veins is higher than the strength of the venous wall. The venous capillaries that drain into these small veins become stretched and twisted. Such veins can protrude above the surface of the skin, and even a minor injury can cause bleeding.</p>
<h2>Reticular varicose veins</h2>
<p>Normally, blood moves through veins from the foot to the lungs and heart with the help of a special valve. Such veins are characterized by elasticity and small diameter; they are not visible or can be seen in good lighting in people with a very thin subcutaneous fat layer and, moreover, with very fair and transparent skin.</p>
<p>Reticular varicose veins are more of an aesthetic problem, which, however, does not pose a health threat and rarely leads to complications.</p>
<p>The diagnosis is very easy – a medical examination is enough.</p>
<p>Conditions for the prevention and control of the development of reticular varicose veins and varicose veins in people with a genetic predisposition: normalization of weight, proper nutrition, active lifestyle; we recommend paying attention to swimming. It is necessary to avoid heavy physical work, lifting heavy objects, prolonged static loads, jumping. We especially note the harm of tap dancing &#8211; strong heel-to-toe blows lead to the rapid development of varicose veins in people with a genetically weak vascular wall.</p>
<p>It is worth noting the role of knitted clothing, such as: anti-varicose stockings and tights for the prevention of varicose veins in patients at risk. Only a doctor can prescribe the wearing of compression stockings of 1, 2 or 3 degrees of compression, based on medical indicators obtained after examining the patient.</p>
<h2>Treatment of reticular varicose veins</h2>
<p>Sclerotherapy. The peculiarities of its implementation are the use by the doctor of a special drug that causes local inflammation of the vein with its subsequent sclerosing and narrowing of the vessels and veins (using small needles that are equal in size to the diameter of the capillary). The thinner the capillary, the thinner the needle. The procedure requires special training and a high degree of technical skill. After this procedure, the venous mesh disappears after some time. Often, one procedure is enough to completely solve the problem. But in the case of a large number of capillaries, several procedures are necessary. <p>Laser treatment for reticular varicose veins: Laser therapy works more slowly than sclerotherapy. It is effective when treating tiny spider veins. More than one treatment may be needed, and the treatment can take about a year for the veins to disappear completely.</p>
<p>These two methods are not opposed to each other, they should be used in combination. After sclerotherapy, laser treatment is performed in order to obtain the best result.</p>
<p>Side effects: temporary skin discoloration in the area where sclerotherapy was performed; blistering; temporary minor discomfort in the areas where spider veins were treated.</p>

<p><strong>Diabetic foot syndrome develops in more than 10% of patients with diabetes. The main reason is the late referral of a patient with diabetes to an endocrinologist. In patients with diabetes, high glucose levels cause destructive changes in the blood vessels and nervous system.</strong></p>
<h3>The long course of the disease leads to serious structural changes:</h3>
<ul>
<li><strong>Cardiovascular system:</strong> the risk of heart attack and stroke increases by 2 times due to damage to large vessels.</li>
<li><strong>Eyesight:</strong> Diabetic retinopathy can lead to gradual or complete loss of vision.</li>
<li><strong>Kidneys:</strong> renal failure develops, which may require hemodialysis.</li>
<li><strong>Nervous system (polyneuropathy):</strong> leg pain, tingling, numbness, and loss of sensation in the extremities appear.</li>
<li><strong>Lower extremities:</strong> due to poor wound healing and impaired blood flow, there is a risk of developing &#8220;diabetic foot&#8221; and subsequent amputation.</li>
</ul>
<p>Patients with diabetes should be examined by an endocrinologist, neurologist and surgeon with a frequency of 1 time in three months to 1 time in a year, depending on the risk group. The above recommendation regarding examination intervals does not apply to those patients who have open wounds &#8211; in this case they should be under constant medical supervision.</p>
<p>Treatment: to prevent the development of lower limb damage, death of nerves, skin, subcutaneous tissue, the development of gangrene, and to treat diabetic foot syndrome, our clinic provides comprehensive treatment.</p>

<h3>Compression, medical hosiery (anti-varicose socks, stockings, tights)</h3>
<p>Compression has long been used to treat varicose veins of the lower extremities and trophic ulcers. <h3>There are compression classes &#8220;A&#8221; and I, II, III and IV.</h3>
<p>Class &#8220;A&#8221; with compression of no more than 18 mmHg &#8211; intended for people with increased static loads on the lower extremities &#8211; flight attendants, surgeons, ballet dancers, dancers, etc. For these categories of people, tights, stockings and knee-highs for tourism and sports should be recommended (to achieve good results).</p>

<p>Separately, between Class &#8220;A&#8221; and 1st degree, and sometimes 2nd degree of compression, there is medical hosiery for childbirth and hospital hosiery &#8211; these are special stockings, tights, intended for patients who are in a horizontal position (lying down).</p>
<ul>
<li>I compression class. As well as medical compression hosiery (gloves, stockings, tights) it is intended for the lower extremities with pressure at the ankle level of 18-21 mm Hg. It is used in the early stages of chronic venous insufficiency (heavy legs syndrome, tendency to edema, reticular varicose veins, telangiectasia), in functional phlebopathy, for the prevention of venous insufficiency during pregnancy, as well as for the prevention of deep vein thrombosis.</li>
<li>II compression class. This includes medical compression hosiery for the lower extremities with a pressure at the ankle level of 23-32 mm Hg. It is used for varicose veins, in particular in pregnant women, for varicothrombophlebitis, for compression therapy after phlebosclerosis and operations on the veins of the lower extremities, as well as for the prevention of deep vein thrombosis in people at risk.</li>

<li>III compression class. The third compression class includes medical compression hosiery for the lower extremities with pressure at the ankle level of 34-46 mm Hg. It is used for varicose veins with trophic disorders and edema, deep vein thrombosis, post-thrombophlebitic syndrome, lymphovenous insufficiency, and for compression therapy after operations on the veins of the lower extremities.</li>
<li>IV compression class. This includes medical compression hosiery for the lower extremities with pressure at the ankle level of more than 49 mm Hg. It is used for lymphedema, congenital phlebodysplasia.</li>

<h3>Compression hosiery for the treatment of edema</h3>
<p>Medical compression anti-lymphatic hosiery can be circular or flat knit. <h3>Instructions for selecting medical knitwear</h3>
<p>The size of the product is selected according to the individual anatomical measurements of the limb. The upper edge of the medical hosiery should be located 5-10 cm above the level of the vein lesion. If the patient is prone to edema, measurements should be taken in the morning, without getting out of bed.</p>

</ul>
<p>Medical compression hosiery of compression class 3 (sometimes) and compression class 4 (always) requires additional measurements and is made to order. The size of the product is determined using the medi size charts.</p>
